They combine pickups within x minutes/hour or so of each other, but the route is different everyday depending on need.

San Francisco. I don't think any hotels in San Francisco have free shuttles. There are some for hotels in the towns near the airport such as Millbrae. It's similar in many large cities. The hotels right by the airport will have a shuttle, but the hotels in the downtown area and resort areas will not.

There are perhaps some to hotels near the airport, but I don't think there are any to the hotels near the tourist area close to the old town, which is about 10 miles from the airport. In looking at the listings for the major hotels, they only list taxis or rental cars as transportation options.
